A Place to Be Proud.ResponsibilitiesPerforms home-based care with family-centered approach.Acts as the on-call nurse for a portion of each month, divided among the nursing team.Practices with a competent and compassionate approach to supporting patients and families facing terminal and life-limiting conditions.Actively participates in the interdisciplinary team and maintains their primary patients' plan of care.QualificationsBSN Required.Ohio Driver's License.Must have excellent critical thinking skills, good communication and customer service skills.Ability to organize and manage time well. Ability to work independently, as well as within an interdisciplinary team.Other necessary skills include, but are not limited to, pediatric assessment, infusion and central line care, injections, wound care and patient education. Must be comfortable with end-of-life discussions and patient experiences.Experience: One year of pediatric experience required; one year of hospice or palliative care experience strongly preferred.
