Purpose of Job This position is responsible for providing homemaking services to home health clients in their personal residence. Job Functions Follows and documents on HMC Care Plan and Service Record. Report patient concerns to Case Manager Performs home management tasks including, but not limited to, cleaning, cooking (meal planning/shopping/preparation), laundry and errands. Performs other duties as assigned.
