Scope
Reviews and monitors therapy and treatment plans.
Perform and document initial and ongoing assessments of patient's condition.
Develop plans describing a treatment strategy, its purposes, and itsanticipated outcome. Plan often includes exercise regimens at home to increaseflexibility, strength, or endurance. May use various modalities to relievepain and reduce swelling.
Provide information regarding appropriate selection/use of adaptiveequipment and support programs. Help to restore physical function, improvemobility, relieve pain, and prevent permanent disability.
Determine patients' ability to be independent and reintegrate into theworkplace or community after injury or illness.
Documents in accordance with home health and regulatory guidelines.
Uses resources as needed.
Promotes mission, vision, and values of Intermountain Health, and abidesby service behavior standards.
Performs other duties as assigned.
May be required to be placed on-call during a regularly scheduled shift.
Minimum Qualifications:
Current/valid state Physical Therapist licensure in state of practice
Bachelor's Degree in Physical Therapy from an accredited school
Basic Life Support (BLS) Certification for healthcare providersendorsed bythe American Heart Association
Current driver's license, insured and reliable transportation and anacceptable driving record
Ability to communicate effectively both verbally and in writing.
Preferred Qualifications:
Previous experience in Home Health, Acute Care or Clinic setting
Doctorate Degree in Physical Therapy, preferred
Physical Requirements:Carrying, Climbing, Crawling,Hearing/Listening, Lifting, Manual Dexterity, Pulling/Pushing,Seeing, Speaking, Squatting/Kneeling, Standing, Walking
