　　From $16.5 per hour

　　Scheduled hours: 8-hour day shift

　　Some evenings and weekends required.

　　Job Description:

　　Provides basic patient care under the direction of a licensed nurse. Serves as patient’s ‘Personal Care Champion’ by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end of life care. Perform visits and/or shift work.

　　Qualifications:

　　Prefer Iowa Certified Nurses Aide

　　Maintains valid licensure/certification/registration if required

　　Current Drivers license and proof of insurance for staff performing visits.

　　Must be comfortable with providing in-home and facility personal cares and have specialized training in end-of-life care.
