Overview The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $7,000 annually Additional compensation for visits over your caseload Job Details Our Home Care Transition Nurses: Establish and maintain professional relationships with key physician groups and referral sources. Act as Liaison to referral sources on behalf of Catholic Home Care. Identify potential market growth opportunities and program expansion. Serve as a Customer service representative and act in a professional and supportive manner at all times. Requirements: Registered Nurse with current licensure in NYS 2 or more years of home care experience preferred Customer Service and Marketing skills required Salary Range USD $90,000.00 - USD $105,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
