Must be able to work full-time, day shifts 7a - 7p or 8a - 8p or 9a - 9p The ideal candidate for this position: * has a minimum of one year experience caring for patients with complex needs, such as dementia and other chronic conditions * is proactive and stays in front of issues by communicating early. * is consistent, dependable, and shows up on time. * understands the importance of documentation; documents each shift. * has a kind heart; shows empathy and compassion. * enjoys learning and is open to new and different ideas/ways of doing things. * will represent the agency with the utmost professionalism. * is able to work at least one weekend per month consistently.
