Description
Must be at least 21 years old.Must have a Bachelor’s Degree in a Mental Health or Intellectual Disabilities or related field.
Experience facilitating admissions to the IDD Waiver State Planning List preferred.
At least 2 years of experience working with people with intellectual and/or developmental disabilities preferred.
Experience providing program oversight in the provision of IDD Waiver services preferred.
Must have a valid driver’s license.
Must be willing to work a flexible schedule occasionally as needed.
Must complete the HCBS training and continued education program
ESSENTIAL JOB FUNCTIONS:1. Facilitate admissions to the IDD Waiver State Planning list.2. Develop relationships with state and regional staff and facilitate the process of requesting referrals and filling open Supervised Living Home beds across the state.3. Coordinate with Program Directors to facilitate transitioning individuals into Supervised Living Home beds – admissions and discharges.4. Assist with coordination of Supervised Living Home staff scheduling and coverage.5. Assist with monitoring Supervised Living Home individual’s finances.6. Assist with hiring for Supervised Living Home staff, as needed.7. Assist the QA Coordinator with the Closed/Overflow records processes.8. Review and monitor service documentation for accuracy and completion and ensure it meets regulatory requirements.9. Become knowledgeable in the Waiver and Millcreek HCBS policies and procedures regarding provision of services and assist to ensure compliance at all times.10. Follow all safety policies and adhere to all worker’s compensation program guidelines.11. Other duties as assigned.
