　　Employment Type: Full-Time Shift: Rotating shifts (6 positions) Hours Per Week: 36 hours

　　Hiring Range : $34.34- $44.64

　　The actual compensation for this position will be determined based on experience and other factors permitted by law.

　　Responsibilities:

　　• Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. • Coordinates with all disciplines for well coordinated patient care under the oversight of the primary RN. • Notes and carries out physician and nursing orders. • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

　　Education: • Diploma from an accredited school/college of nursing required within 6 months of Date of Hire.

　　Complexity of Work: • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

　　Personal Protective Equipment: • Follows Standard Precautions using personal protective equipment as required.
