The MetroHealth System Department of Geriatrics is seeking an experienced APRN or Physician Assistant (PA) with a passion for critical care, process improvement, and excellent quality of care to work in our Red Carpet Care program. The Red Carpet program provides care to our medically complex, at-risk Medicare patients in their home using an interdisciplinary approach.

　　Through our Red Carpet Care Program, the Population Health Innovation Institute improves outcomes for our complex high risk population. Red Carpet Care is a program that cares for chronically ill and/or frail patients. It works in parallel with primary care to proactively manage chronic conditions in multiple ambulatory settings with individualized care plans.

　　· There is an on-call component

　　· Provider should have experience with home visits

　　Position Requirements

　　Required:

　　· MSN or grandfathered under Ohio Revised Code Graduate of an accredited advanced practice registered nurse program or a PA

　　· Current Ohio licensure as a Registered Nurse and Advanced Practice Registered Nurse from the Ohio Board of Nursing. National certification as Advanced Practice Registered Nurse in specialty

　　· Current Basic Life Support (BLS) from the American Heart Association or obtains within 90 days of hire

　　· Obtains other advanced life support certifications applicable to practice area within 180 days of hire Meets the requirements/qualifications for clinical privileges in the specialty/department with The MetroHealth System and must report to Medical Staff Office at time of hire to apply for clinical privileges Maintains a standard care arrangement and renews/signs it as required by the Ohio Board of Nursing

　　o Note: Other department specific qualifications for providers will be addressed through the Certificate of Authority and credentialing process.

　　Preferred:

　　· 2 years Nurse Practitioner experience

　　· Experience in primary care, family practice, internal medicine, geriatrics, ICU, or ER, chronic care

　　· Bilingual (Spanish)
