Responsibilities:
Responsible for intervention and treatment process for families referredfor or seeking In Home Family Services.
Provides screening and assessments in order to identify familyprotective and risk factors, challenges, strengths, and areas to addressin the service plan.
Provides services and support in conjunction of the service plan; withconstant review and adjustment as needed.
Maintains expected level of direct contact with families per programrequirements, and remains available to families 24/7, for crisisintervention as needed.
Maintain complete, accurate, and current service reports on familiesserved including timely written case notes, treatment plans, familyassessments and court reports as required and/or requested by DCFS,referral sources and SOS.
Conducts audits of client files at least quarterly.
Provides training and support groups on various topics, as needed, basedon community and client trends, requests from organizations, andcollaboration with other programs. Topics include, but are not limitedto, healthy relationships, self-esteem, bullying, financial literacy,leadership, parenting, and more.
Develops and maintains effective working relationships with communityservice providers and resources, such as medical staff and clinics,public welfare personnel, lawyers, therapists, law enforcement staff,school officials, neighbors, religious and youth organizationrepresentatives, etc.; utilizing referral sources as necessary.
Coordinates effectively with families and community agencies for themaximum utilization of available resources to address the service goalsof the cases assigned.
Prepares and submits reports and/or recommendations to the directsupervision concerning case load status, budget requests, and changes inservice plans, and any unusual incidents that occur in the operation ofthe program. Perform on-call duties as needed to carry out theresponsibilities as required.
Communicates problems and issues within the department to the In HomeFamily Coordinator or Director.
Must be able to function independently and have flexibility, personalintegrity, and the ability to work effectively with children andfamilies, staff, and support agencies.
Completes all mandatory agency and job specific training.
Represents SOS in a professional manner to the public and guests.
Complies with all SOS policies, state, federal, county and city laws,rules and regulations.
Performs other duties as assigned.
