Mission of the Family Advocate: Support families through community partnerships, frequent communication, records management, and follow-up on areas of need to proactively assist families in working toward their goals related to health, education, and self-sufficiency.
Typical Duties
Establish and maintain an outreach and recruitment process, ensuring enrollment of eligible children for the assigned service area.
Maintain knowledge of all community programs, establish relationships with community providers, and make referrals to families.
Assist families in utilizing community resources, including medical, dental, social, educational, and occupational services.
Develop Family Partnership Agreements with families.
Document and track follow-up needs related to Family Partnership Goals, as well as needs related to health, dental, nutritional, hearing, and vision screenings.
Assist family with insurance questions and help with arranging follow-up care, medication, aids, and equipment.
Encourage parent participation in center-based programs and activities and document all volunteer activities.
Conduct parent meetings with families monthly.
Conduct home visits on the prescribed schedule.
Monitor attendance of enrolled children daily and contact the family when a child has been absent for two consecutive days.
Conduct home visits per the established timeline and document all family contact.
Work with families to insure children receive all required screens and exams within the prescribed timeframe.
Provide specialized translation or family support services as appropriate and able.
Collect all documentation from parents and staff and enter all completed forms into the appropriate database.
Update all data in databases in a timely manner.

Requirements
The Family Advocate must have, at a minimum, within eighteen months of hire, a credential or certification in social work, human services, family services, counseling, or a related field.
Must have two years or more of related experience.
Must pass criminal background check in MD and surrounding states.
