Summary:
The Family Advocate will work as part of Cook Children's Physician Network to provide support and resources to families for issues that extend beyond their clinical needs. Duties include:
Build individual and family capacity by increasing health knowledge and self-sufficiency.Responds to the social and emotional needs of our families, providing guidance and support while navigating them through the health care system and community to achieve the best health outcomes.Serve as both an advocate and liaison between the patient/family, health care team, and community partners.Functions in collaboration with the treatment team within the Cook Children's Neighborhood Clinics.
Hours: Mon-Fri 8am-5pm
This position will provide coverage to 2 clinics. All neighborhood clinics are located in Tarrant County
Required Experience & Education:
3 years' experience working in health care or working with vulnerable populations.High School diploma or equivalentStrong verbal and written communication skillsStrong customer service, phone, and computer skillsFamiliarity with basic medical terminology.Highly organized with the ability to prioritize and demonstrate problem-solving skills.Ability to work autonomously with minimal supervision.
Preferred Experience, Education & Skills:
Experience working with family and social services.Experience providing psychosocial assessments & crisis intervention.Bachelor's degree in Social WorkBilingual: Spanish & English
