4) Rate of Pay: $23.64 per hour

6) Employer will use a single workweek as its standard for computing wagesdue.

8) Does Employer offer any type of bonus or pay above the hourly wagerate? At discretion of Employer, yes bonuses may be provided.

10) Employer will make all deductions for workers' paycheck as required bylaw; deductions Employer intends to make from paycheck, which are notrequired by law, if applicable, would-be deductions for housing, ifemployee choose voluntary housing option. Employer may allow deductions notrequired law as long as advance permission is granted by employee or Employerwill state the specific deductions.
11) If the worker completes 50% of the work contract period, employerwill reimburse the worker for transportation and subsistence from the place ofrecruitment to the place of work. Upon completion of thework contract or where the worker is dismissed early, employer will provideor pay for the workers' reasonable costs of return transportation and dailysubsistence back home or to the place the worker originally departed towork, except where the worker will not return due to subsequent employmentwith another employer. The amount of transportation payment or reimbursementwill be equal to the most economical and reasonable common carrier for thedistances involved. Daily subsistence will be provided at a rate of $15.46per day during travel to a maximum of $59.00 per day with receipts.
12) Employer will pay for the worker's cost of return transportation anddaily subsistence from the place of employment to the place from which theworker, disregarding intervening employment, departed to work for theemployer, if the worker completes the certified period of employment or isdismissed from employment for any reason by the employer before the end of theperiod, as is also outlined above.
13) Employer does provide daily transportation to the worksite.
14) Employer will reimburse H-2B worker in the first workweek for allvisa, visa processing, border crossing, and other related fees,including those mandated by the government incurred by the H-2B worker viacheck issued to employee (but need not include passport expenses or othercharges primarilyfor the be efit of the worker).
15) Employer will provide workers, without charge or deposit charge, alltools, supplies, equipment required to perform duties assigned.
