POSITION DESCRIPTION: This position is responsible for the CaseManagement of all group employment referrals and members. Responsiblefor ensuring that individuals served in the department obtain andmaintain meaningful employment within the community. Perform all casemanagement tasks including but not limited to completing progresssummaries, assessments and goals, attending ISP meetings, completing jobevaluations and creating career plans. QUALIFICATIONS: Associate degreewith a minimum of 1 year experience in a supported employment program orHigh School diploma/GED with 4 years? experience working withdevelopmentally disabled adults in a supported employment program or ina position of case management. Ability to complete all initial andongoing background checks including but not limited to: CORI, SORI,Fingerprinting, LEIE, driving record, E-verify, Physical, PPD andlicensing and/or certification checks (i.e., nursing). Valid US driver?slicense and ready access to a vehicle. TRAINING REQUIREMENTS: Mustmaintain all certification needed in the program including CPR, FirstAid, ), and BFAIR Human Rights Training. May need to attend workshops,seminars or BFAIR functions as needed for job development.RESPONSIBILITIES: 1. Treat each individual, co-worker, supervisor andothers with the respect and dignity due every human being and adhere tothe Department of Developmental Disabilities (DDS) Human RightsStandards and the Disabled Persons Protection commission?s (DPPC)Mandated Reporter Guidelines around physical and emotional abuse,including timely reporting violation. 2. Ensure program meets qualityassurance and certification standards by monthly review of necessarydocumentation and routine participation in and/or observation ofservices provided. 3. Work with community business to develop employmentopportunities for people served and for vocational evaluation purposes.4. Work individually with people as indicated, including providingphysical assistance and/or supervision with mobility (light and heavylifting and transfer), assist with hygiene, task training, recreationalactivities, or other needs. Provide coverage at sites as needed inaccordance with the contents of each individual?s ISP, medicationtreatment plan and documented behavioral interventions. 5. Managereferral and intake process in conjunction with funding sources. Developand maintain positive working relationships with other funding sourcesby working collaboratively ensuring open and consistent communication.6. Oversee assigned work sites 7. Complete all facets of ISP process 8.Oversee goal and objective development 9. Oversee goal and objectivedevelopment 10. Complete other ISP documentation 11. Meet weekly withAssistant Director; attend staff meetings and other meetings as needed.12. Participate in Office of Quality Enhancement (OQE) preparation toensure the department meets all standards to maintain licensure andcertification. 13. Ensure implementation of all agency policies andprocedures as specified in agency personnel policies handbook andprogram policies and procedures manual. 14. Act as an appropriate rolemodel through dress, speech, and actions. 15. Ensure all regulatorystandards are met including staff ratios, safety plans, fire drills,tests of emergency plans, semiannual site inspections, and equipmentmonitoring.
