　　Sign-on bonus: $1,500.00

　　Schedule: Full time night shift either 3, 8 hour shifts + 1, 12 hour shift or 3, 12 hour shifts per week. 8 hour shifts are 2245-0715, 12 hour shifts are 1845-0715. Every other weekend. Holiday rotation. Orientation schedule may vary.

　　Responsibilities

　　Perform basic clinical and non-clinical patient care activities under supervision.

　　Aid with movement and placement of patients.

　　Assist patients with tending to personal care and activities of daily living.

　　Report findings or changes in physical, mental and emotional conditions to nursing staff.

　　Document necessary records.

　　Keep patient rooms clean, orderly, and maintains medical equipment.

　　Perform emergency department specific care/duties as assigned within scope of job.

　　Requirements

　　Licensure / Certification / Registration:

　　BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

　　Nurse Aide credentialed from the state of Wisconsin obtained prior to hire date or job transfer date required. Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date. Additional certs may be required by specific job locations.

　　High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

　　Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
