JOB REQUIREMENTS: Summary of Position Responsible for working withindividuals to support them to obtain independent community living. Thisposition will mentor and build relationships with individuals to assistthem to connect to their communities and live more independently.Education and Experience High school diploma or equivalent. At least two(2) years of experience working with individuals with disabilities or ina mentor/teaching position that assists individuals in learning skillsand providing them independently. Individual demonstrates experience andunderstanding of community resources and methods of teaching independentliving skills. A valid driver\'s license with good driving record andinsured vehicle available for daily use. This position requires annualbackground checks. Knowledge, Skills and Abilities/Essential Functions**Following is a listing of representative examples of duties for thisposition; it is not considered to be a complete listing of all dutiesand responsibilities for the position. Maintain a professional imagewhile working in the community and in individuals place of residence.Assist individuals with any activities of daily living that are neededto live independently. This can include but not limited to cleaning,cooking, laundry, medication support, bill paying, and personal caresupport. Develop community relationships that connections can be madebetween the person supported and community organizations (volunteer, orjoin a club as example). Make decisions and work independently. Buildstrong relationships with the individuals that you work with to assistin learning and obtaining new skills that support continued movementtoward more independence. This position does not provide a support(example cook for someone) but instead teaching for the purpose of theindividual gaining independence. Communicate effectively in person andfor record keeping. Work cooperatively with all people. Work a flexibleschedule some evening hours and weekend work. Will be required to be oncall 24/7 on a rotating schedule within the team. Document outcomes andaction steps for each individual supported. Break tasks down toachievable components. Responsible to schedule and complete workassigned for the position. Maintain working relationship with communitypartners to assist with community connections for individuals wesupport. Perform creative problem solving as needs arise for individualin the community and in their home/apartment Work cooperativelyCommunity Support team and manager to develop and update outcomes andaction steps for successful support. Work one-on-one with individuals orin small groups on a daily basis. Implement personal profile plans on adaily/weekly basis to meet the individuals outcomes. Willingness tocomplete first aid and CPR training and certification if requested to.Case management III. Ability to demonstrate, train and/or model jobtasks/duties/responsibilities. Ability to perform quality inspectionaccording to ODC\'s and/or customer\'s requirements. Mobility to accessvarious locations within/outside ODC facilities. Physical ability tolift or transfer adults when providing personal care. Physical abilityto provide and/or respond to behavior intervention. Lifting 25 35 lbsfrequently up to 60 lbs occasionally.
