THE DUTIES:
Addresses the ancillary needs of individuals participating in treatment, including MAT coordination and medication monitoring. Conducts intake interviews in the Ridgway and St. Mary's offices, utilizing appropriate assessment practices to obtain necessary information from clients, families, and/or involved collaterals for the identification of service needs for drug and alcohol related problems.Determines level of care based on placement guidelines.Determines appropriate funding arrangements and authorizes or recommends services for alcohol and other drug clients.Orients clients to the program, policies, procedures and treatment process.Updates the clients service plan as needed.Provides case coordination services to determine client non-treatment needs that need to be addressed as part of the recovery process and to ensure access to available alcohol and other drug and ancillary services, such as housing, child care, medical and financial services.Monitors client treatment progress and/or utilization review activity.Prepares written reports and maintains client records utilizing Electronic Medical Records.Administers urine screens as necessary.Serves as an advocate for clients.Acts as liaison between prescribing physicians and DA treatment providers for clients receiving medication assisted therapies.Acts as a liaison between Penn Highlands Elk, ADAS, and all DA treatment providers.Facilitates the warm hand off process (WHO) to all individuals in care at Penn Highlands with a desire to obtain treatment services.Travels by means of own transportation. Overnight travel may be necessary at times.Must be flexible and available to travel to other office locations for coverage as needed.Performs all other duties as related to the Case Management departmentTHE REQUIREMENTS:
Knowledge of social casework principles and methods.Knowledge of addiction as a disease and its treatment.Knowledge of current social, economic and health problems and available resources.Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds.Ability to communicate effectively, both orally and in writing.Ability to work with persons who have physical, mental or emotional disabilities or who are economically disadvantaged or involved with the criminal justice system.MINIMUM EXPERIENCE AND TRAINING:
One year as a Drug and Alcohol Case Management Specialist Trainee;
Or: A Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting;
Or: A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting.
Or: Any equivalent combination of experience and training.
