Job Summary
Under minimal supervision, performs technical Public Health work in thedetection and treatment of communicable diseases. Responsible for workingwith, testing and counseling persons exposed to, infected with, orhaving a positive test for communicable diseases in a variety of clinical,community, business, institutional, and home settings. Work alsoinvolves providing information about the disease and course of treatment,ensuring care plan adherence, providing appropriately prescribed treatmentand/or prophylaxis, collecting and reporting medical, psychological andsocial information to physicians and nurses, health departments andlaboratories, and facilitating the linkage of clients to appropriate stafffor additional care and services as required .
This position is with the HIV/STI Prevention in Outreach program at thePublic Health Institute at Denver Health. The position provides HIV and STItesting services in non-clinical settings and therefore evening and weekendavailability is mandatory. The HIV/STI Prevention in Outreach programstrives to ensure our services are offered in an equitable and affirmingmanner to the diverse communities it serves.
Essential Functions:
Field Work and Clinic Setting:* TESTINGTriages, registers, tests, delivers test results and provide crisiscounseling or brief therapy techniques, documented by completion ofappropriate in-service training.Uses standard precautions for specimen acquisition and storage, includingphlebotomy.Uses basic laboratory techniques necessary for the successful implementationand interpretation of clinical tests.Interpret test results according to current clinical standards and explainresults to the patient accurately and appropriately for that individual,including conveying difficult information about serious and complex illnesses.* COUNSELING, EDUCATION, AND FOLLOW-UPPossesses a working knowledge of diseases/conditions specific to the areaand interventions related to them.Conducts pre- and post- test counseling to clients who present for testing.Assists patients to identify barriers to effective risk reduction and healthpromotion.Works with clients to identify modifiable behaviors and other issues impactinghealth and have ability to refer them to available resources to help withthese issues.Facilitates client access and transition to follow-up services as indicated.Provides referrals for Denver residents to the Medicaid, CICP, DFAP, andChild Health Plan program enrollment staff.Participates in care coordination, extended to settings outside the clinicthat require good professional working relationships with various communitybased organizations and agencies serving the same clientele.Accurately and clearly articulates to clients, health care staff,community members, organizations, and businesses basic public healthprinciples and standard of care to protect public health and to prevent thespread of specific communicable diseases.Recognizes when assistance from other staff is needed for the implementationof treatment, interpretation of tests, and follow up care.Readily recognizes and reports problems or untoward events related totesting, medication and/or treatment.(80%)Accurately and appropriately documents all patient encounters, includingentry of required data into medical records and other tracking programs,including:* Uses of electronic resources and maintain data bases to identify individualsrequiring coordinated outreach and intervention fro multidisciplinary team.* Collects, organizes and analyzes data to perform specific Public Healthactivities for communicable disease prevention* Articulates and presents findings and data through verbal, written andelectronic communication(15%)Observes work rules of the agency and department.
