About Discharge Planner

  Description

  This position reports to the Director of Case Management or designee. The Discharge Planner collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The Discharge Planner also proactively escalates risks and barriers to discharge to the Case Manager or Social Worker; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.

  Required Qualification:

  Completed an LVN program from an accredited school Or

  10 years or more care management experience

  Completed Bachelors degree

  California Fire and Life Safety Card - National Organization within 30 days of hire

  2 years Experience in health care setting (acute or post-acute)

  2 years LVN experience (if LVN)

  2 years Experience in health care case management discharge planning.

  The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
