Hourly Rate: $17.20-$20.20
Duties and Responsibilities:
Remain alert and responsive while on scheduled shift(s).
Facilitates active participation of the individuals by implementing their habilitation plans in their family home, respite and/or out in their community. Completes the necessary training to be able to work closely with fellow staff, as well as, work one-on-one with an individual in an isolated environment within their family home, respite, various day habilitation worksites and/or out in the community. Responsibilities include, but are not limited to, engaging in necessary lifting, moving and positioning tasks; facilitating improved gross motor, fine motor and range of motion skills; promoting self care, activities of daily living and personal hygiene; fostering receptive and expressive communication through many different avenues such as, sign language, pictograms, electronic devices, etc; assisting with meals through modeling appropriate behavior, therapeutic feeding and use of therapeutic equipment; assisting with recreational activities and facilitating active engagement in community outings; accurate and consistent implementation of behavior support plans/SCIP-r.
Navigates the Agency's IntrAnet, time tracking program and the Agency's email system. Regularly checks the IntrAnet, time card and email and is responsible for all information shared in written and electronic form. Approves time card according to the payroll schedule and responds to email requests in a timely manner.
Accurately and consistently completes all required documentation practices, in a contemporaneous manner, utilizing the PrecisionCare software and in written form, when necessary.
Facilitates and/or provides for the personal and physical care needed by each individual including tasks such as bathing, toileting, eating and dressing.
Performs housekeeping tasks in respite as designated by the shift supervisor/Residence Coordinator including tasks such as meal preparation, cleaning, laundry and shopping.
In keeping with confidentiality and HIPAA practices, communicates a wide range of information regarding the residents to a variety of people in and out of the waiver services program such as co-workers, teachers, supervisors and parents.
Is responsible for following the Code of Ethics for Direct Support Professionals, as well as, show proficiency in the Core Competencies to ensure person-centered supports for the individuals participating in the waiver service programs.
Attends and actively participate in, at least, biweekly staff meetings as scheduled or requested by the Staffing Supervisor and/or Waiver Services Manager.
Attends and actively participates in case reviews, in-service sessions, team meetings and other meetings upon request.
Implements fire, safety and emergency procedures and assists with fire drills when working in respite.
Implements and follows all required individual programming and Agency/Program-specific protocols and procedures accurately, consistently and according to how they are written.
Safely drives Agency vehicles and personal vehicle, when necessary, to assist in transportation to and from day habilitation activities, recreational activities, community events and other appointments as required.
Maintains Agency-Approved Driver status, as well as, certifications in SCIP-r, CPR and First Aid. Completes all required ongoing/annual trainings and in-services, as assigned.
Qualifications, Experience and Training:
Applicant must be able to demonstrate they have achieved a Local/Regents High School diploma or Equivalency Diploma issued by a local school district or State Education Department. This achievement will be verified directly with the issuing entity. Considerations will be made for applicants who do not possess a high school diploma or equlivalent. Six (6) months to one (1) year related experience and/or training is desired.
Must maintain Agency-Approved Driver status, certification in CPR, First Aid, SCIP-r and all other training mandated by OPWDD regulations.
Must have the ability and temperament to work effectively with youth and young adults who are developmentally disabled. Must be able to work alone and in a team oriented environment, possess strong communication skills including communicating effectively amongst staff, exhibit patience, handle flexibility in scheduling, etc. Must have the ability to take part in various physical and recreation activities outside the residence.
Ability to read and interpret documents such as safety rules, policy and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of people. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Working Conditions:
While performing the duties of this job, the employee is frequently exposed to working in a variety of environments including, but not limited to, family homes, certified group homes, certified foster care homes and in various venues out on the community. These environments are very diverse in nature. While performing the duties of this job, the employee may be frequently exposed to individuals who engage in aggressive, destructive and attention-seeking behaviors that may involve exposure to body fluids. The employee may also be exposed to body fluids while bathing, dressing and/or assisting individuals in the bathroom or when they are ill. The environment is climate controlled. The employee is required to participate in travel and transition activities; therefore, exposure to various outside weather conditions exists. The noise level in the work environment is usually moderate.
Physical Requirements:
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk and hear. The employee frequently is required to sit, stand, walk, stoop, kneel, crouch, or crawl. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The physical demands of the job will vary from site to site. Staff working in a residential setting may be engaged in very physical activities with the residents; which include, but are not limited to, biking distances of up to 3 miles at a time, hiking/climbing at local parks and trails, swimming, and pushing a wheelchair for periods of up to an hour while on outings. Staff may need to assist with lifting and transferring of individuals and/or items requiring some lifting and carrying. The employee must regularly lift and/or move up to 50 pounds repeatedly and have the strength to resist force for extended periods of time. Mary Cariola Center uses SCIP-r approved physical interventions as part of some individuals behavioral support plans. Staff will need to be able to sit on their knees for up to 20 minutes in both indoor and outdoor settings.
