　　Job Description/Essential Functions:

　　Horizon House is seeking a Direct Support Professional (D.S.P.) with experience working with medically fragile individuals in a residential/community setting. D.S.P. provides support with daily living activities, community integration, medical appointments, religious events, and the development of Individual Service Plans (ISP) to individuals experiencing Mental Health/Intellectual Disabilities diagnosis. A D.S.P. will find assisting individuals with learning to live and function as independently as possible in the community rewarding.

　　Job Qualifications:

　　High School diploma or GED and at least 2 years prior experience in the MH/IDD field or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.Good communication skills, both oral and written.Ability to perform independently and as a team member.Ability to use a computer and document daily activities electronically.Required to possess a valid PA driver's license and maintain an acceptable driving record.Horizon House is an equal opportunity employer.
