　　Responsibilities

　　Educate patients, healthcare workers and the community about controlling diabetes through nutritional management.

　　Coordinate with designated staff to effectively integrate medical nutrition therapy into the overall management of diabetes.

　　Provide diabetes education for patients, healthcare workers and the community.

　　Encourage the diabetes team to support and reinforce nutrition prescriptions and to help patients manage their meal plans.

　　Supervise the planned education process for the goals of medical nutrition therapy for diabetes. Reinforce diabetes education and instruction to the staff and implement and monitor educational plan of care.

　　Coordinate with healthcare providers including receiving and writing orders to ensure effective and efficient patient care and achievement of desired patient outcomes.

　　Requirements

　　Licensure / Certification / Registration:

　　Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtained prior to hire date or job transfer date required.

　　Educator specializing in Diabetes Management preferred.

　　Education:

　　High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.Additional Preferences

　　Licenses or certification issued by Texas Board of Dietitians.

　　Minimum 2 years clinical experience preferred.

　　Bilingual preferred, not required

　　2 years of experience in RDN or RN role

　　CDCES certification preferred or completed within 2 years of employment.

　　Experience with pattern mgmt [BG logs, CGM reports and insulin adjustments by CDCES] highly favored

　　Experience with insulin pumps and CGM highly favored
