JOB REQUIREMENTS: Responsibilities OverviewScreens and assessesnutritional status of patients / residents. Develops and implements careplans, coordinating services with medical staff and ancillary services.Participates in clinical Quality Improvement activities for thedepartment. Enters clinical charges into the computer. Providesnutrition education for patients / residents. Represents the HealthSystem as a nutrition resource person. Performs other duties asassigned. Essential Functions- Screens and assesses patientsa status fornutritional risk factors per protocol. This may include gatheringinformation on the nutrition history, medical record review,anthropometrics. - Prepares Nutrition assessment based upon informationobtained from history, including recommendation of appropriate dietprescription. Plans nutrition care goals, completes timely follow-up anddocumentation in the medical record. Develops and implements plan ofcare based assessment of nutritional needs and available sources, andcorrelates plan with other healthcare. Evaluates nutritional care andprovides follow up continuity of care. Consults with physicians andother health care professionals to determine patientas nutritional needsand diet restrictions. - Organizes daily tasks to complete neededactivities in a timely manner. Participates as a team player with therest of the clinical nutrition staff by communicating effectively andidentifying priority duties. Participates in clinical meetings todiscuss policy and procedures and other topics. - Participates ininterdisciplinary care rounds by providing input regarding patientnutrition status. Discusses nutrition related problems with medical,nursing, and ancillary staff to improve nutrition status of assignedpatients. - Maintains accurate records and assists in statisticalanalysis of data. Assists in developing new programs including businessplans as needed. Assists in developing policies and procedures asappropriate. Completes data for Clinical Productivity monthly. -Participates in food tasting panels, test trays, meal rounds, etc. toachieve / maintain above average ratings on patient satisfactionsurveys. Inspects meals served for conformance to prescribed diets andfor standards of palatability and appearance. - Provides nutritioneducation to patients, the community, students and other health careprofessionals. Develops nutrition education materials, programs,displays, etc. - Maintains CEUas as required for registration andlicensure. Attends Mandatory Inservice and completes online education asrequested. Minimum QualificationsBacheloras degree from a four yearcollege or University in Dietetics, Food Service, Food and Nutrition orrelated.
