POSITION DESCRIPTION: The Day Hab RN/LPNRelief is primarily responsible for providing various medical supportsto individuals with disabilities in a Day Habilitation program. TheRN/LPN provides medical treatment to individuals as needed and isresponsible for recordkeeping and communicating with medical providersand advocating for the medical needs of the people we support.QUALIFICATIONS: Registered Nurse Degree or Licensed Practical Nursingdegree required with at least one year of time involved in providingnursing supports in community services for individuals withdevelopmental disabilities, as well as current license in the State ofMassachusetts. Ability to complete all initial and ongoing backgroundchecks including but not limited to: CORI, SORI, Fingerprinting, LEIE,Driving record, E-verify, Physical, PPD and licensing and/orcertification checks (i.e. nursing). Valid US driver\'s license andready access to a vehicle. TRAINING REQUIREMENTS: Receive and maintaincurrent certification in First Aid, CPR, Safe Techniques to AvoidRestraint and Training (START), and BFAIR Human Rights Training. Mayneed to attend workshops, seminars or BFAIR functions as needed for jobdevelopment. RESPONSIBILITIES: 1. Treat each individual, co-worker,supervisor and others with respect and dignity due every human being.Adhere to the Department of Developmental Services (DDS) Human RightsStandards and the Disable Persons Protection Commission\'s (DPPC)mandated reporter guidelines around physical and emotional abuse,including the timely reporting of violations 2. Complete daily nursingnotes as warranted, monthly nursing notes, quarterly health assessments,and monthly vital signs. 3. Work collaboratively with the Health CareSupervisor in the oversight of medication administration practices asper agency policy and governing regulations. 4. Monitor/order medicaland infection control supplies through the Program Director. 5. Act as aliaison with the individuals served, parents and/or guardians, otherservice providers, physicians and/or medical resources, as well asbetween day and residential staff members, etc., to promote optimalprogramming and quality services. 6. Respond to all accidents, injuriesand emergencies and recommend appropriate interventions such as ERvisits, doctor\'s visits and going home. 7. Prepare any special reportsas requested; meet weekly with Program Director and/or RN Supervisor. 8.Consult with hospital staff, family and or guardians, RN and otherProgram Directors as applicable on individual\'s medical concerns andreview with program staff as warranted. 9. Maintain accurate andcomplete medical records, reports, and data necessary to meet therequirements of DMA, DDS, DPH, and BFAIR. 10. Adhere to agency/personnelpolicies, DMA, DDS, DPH regulations, follow emergency and call-inprocedures, and maintain confidentiality.
