　　What you will be doing in this role?

　　Congenital Heart Program (CHP) clinical social work position provides vital social work services including assessment, intervention, ongoing counseling, support and follow-up for patients and family members across the continuum of care. This social worker connects patients/families with appropriate support programs, community based organizations, and mental health resources. Social Worker provides crisis intervention as deemed necessary and assists with care coordination, regional center referrals, advocacy and communication with CCS organization.

　　Crisis intervention.

　　Individual and family counseling.

　　Discharge planning and case coordination

　　Interdisciplinary discussion of psychosocial implications of illness including consultation regarding treatments and resources designed to solve the problems of the patients.

　　Develops and maintains productive relationships with community health, welfare, and social agencies

　　Initiates program planning and development in assigned services/programs.

　　Part time position – 8am to 4:30pm

　　Qualifications

　　Master’s degree in social work required.

　　1 year in acute care hospital setting or related medical clinic experience - preferred
