  THE POSITION

  DESCRIPTION OF WORK

  This professional social service position is assigned to the Adult Targeted Case Management (TCM) Services Unit of the Lycoming-Clinton MH/ID Program and will follow the Community Based Model. This worker will function as a Blended Case Manager with a caseload of up to thirty (30) consumers who have severe and persistent mental illness.

  The Case Manager will assist consumers in obtaining necessary resources in the community to work towards their recovery goals and will work with the consumer as long as necessary in brokering resources and consulting with families and significant others. This position will be supervised by the Adult Targeted Case Management Unit Supervisor.

  Work Schedule and Additional Information:

  Full-time employment.

  Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.

  A weekly on-call rotation schedule will be utilized in order for consumers to have 24 hours per day, 7 days per week available services.

  REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

  QUALIFICATIONS

  Minimum Experience and Training Requirements:

  Six months of experience as a County Caseworker 1;or

  Successful completion of the County Social Casework Intern program;or

  A bachelor's degree with a social welfare major;or

  A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency;or

  Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.

  Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

  Other Requirements:

  PA residency requirement (https://www.employment.pa.gov/Additional%20Info/Pages/default.aspx) is currentlywaivedfor this title.

  Hiring preference for this vacancy will be given to candidates who live within Lycoming, Clinton, Northumberland, Sullivan, Columbia, Montour, Union, or Centre counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
