About Community Health Worker – Internal Medicine

  Description

  The primary care Community Healthcare worker coordinates services and guides the patient across the continuum of care to help achieve the best possible quality, experience and affordability outcomes. The CHW works as part of an integrated care team that includes providers, clinic staff, social workers and hospital staff to effectively address the needs of patients in a collaborative, patient centered environment. The CHW has a solid understanding of the various Providence and community provided resources and can advise patients on how to best take advantage of these resources.

  Required Qualifications:

  Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.Preferred Qualifications:

  Experience as CHW.

  3 years' Work experience with healthcare and/or customer service role.

  The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
