Health and Human Services provides essential services to individualsthroughout Coconino County, including through the Pathways to CommunityRe-Entry Program located adjacent to the Coconino County DetentionFacility. The Pathways to Community program is a collaborativepartnership between the Coconino County Sheriffs Office and CoconinoCounty Health and Human Services (CCHHS) that provides Re-Entry supportfor individuals leaving the Coconino County Jail. Individuals beingreleased from custody will receive immediate connection to social,behavioral, housing, transportation, and legal services.
The Pathways to Community Community Health Worker I position will engagewith clients as they are booked into the Jail to conduct screeningsidentifying the needs of the clients to begin the re-entry planningprocess. This position will be responsible for engaging clients as theyare booked in to the jail, and re-engaging them as they are released,initiating the process of care coordination and providing resourcenavigation support. This position will work as part of the Pathways toCommunity team, ensuring effective coordination of re-entry services forclients. This position will operate on site with no remote workopportunities.
Under general direction, is responsible for entry level professionalcommunity health work in one of several health and human servicesprograms. Serves as a liaison between Health and Human Services and thecommunity to facilitate access to services and improve the quality andcultural proficiency of service delivery. Works to improve healthequity. Conducts outreach to implement programs in the community thatpromote, maintain, and improve individual and community health; performsrelated work as assigned.
