Reporting Relationship: Reports to the Community Cares Director
Supervision Responsibilities: No
Essential Role: Affect improvements in the overall health of individualsseeking Community Cares services by providing professional caremanagement & coordination services. The Community Cares model of servicedelivery supports the provision of timely, comprehensive, high-qualitychronic care management services that operate under a whole personapproach to care. The whole-person approach to care addresses all of theclinical and non-clinical care needs of the individual.
Duties and Responsibilities
Referral tracking.
Training and technical assistance.
Data management (including processing enrollment/discharge/transferforms).
Scheduling for primary care community care team and enrollees.
Chart audits for compliance.
Reminding enrollees regarding keeping appointments, fillingprescriptions, etc.
Requesting and sending medical records for care coordination.
Qualifications:
Education/Certifications/Licenses/Registrations
High school diploma or equivalent.
Bachelors of Science in counseling, sociology, psychology or social workpreferred.
Valid drivers license and reliable transportation for outreach travel.
Bilingual proficiency in Spanish and English is required.
Experience
Prior office and/or medical setting experience.
Previous case management experience preferred.
Previous community health or FQHC experience preferred.
Technical Skills
Must possess a thorough knowledge of modern office practices,procedures, and equipment, including computers, copiers, and otherstandard office equipment.
Ability to work independently, organize, monitor, and adjust work asnecessary to ensure accuracy and timeliness.
Demonstrated ability to identify and solve problems.
Excellent organization skills and commitment to accuracy.
Ability to collaborate and interact with a diverse group of health careprofessionals.
Excellent analytical skills necessary for managing the systems of careprovided in a clinical delivery environment.
Behavioral

Must display good verbal and written communication skills, and be ableto professionally receive and follow oral instructions.
Work Schedule:
Normal schedule is Monday through Friday 8:00 am to 5:00 pm., schedulemay vary.
Working Conditions:
Medical office setting. Ability to sit and/or stand for long periods oftime. Bending, stooping, and lifting (up to 50lbs.) may be required.Interaction with physicians, patients and other office personnel. Mayinvolve contact with angry, upset or ill persons. May involve potentialexposure to blood and body fluids and other hazardous substances. Musthave visual acuity and manual dexterity to interface with computer. Musthave auditory acuity to handle phone calls. Light travel required.
