The Community Care Coordinator (CCC) works as part of Champ?s care teamthat provides supportive services and case coordination to addressparticipant needs, act as liaisons between community organizations onbehalf of Champ Homes, and its participants, consistently demonstratesprofessional conduct, and ethical practice while promoting an atmosphereof self-advocacy. The CCC?s perform a variety of case managementservices that include, but are not limited to, coaching, referrals, andadmission functions. All Care Coordinators and Support Staff, who makeup the care team, report to the Lead Community Care Coordinator. CCC?sare responsible to report progress and challenges to the LCCC in atimely fashion. The LCCC will provide guidance and support to the team.DUTIES AND RESPONSIBILITIES: DIRECT CARE Assist with referrals andprogram admissions following Champ?s admission criteria and intakeprocedures, completing required documentation. Coordinate participantmove ins and program discharges according to admission criteria.Coordinate participant move outs and the clean-up of rooms, includingthe moving of furniture as needed. Work as a part of the care team toprovide supportive services and case coordination to address participantneeds. Meet with each participant on caseload, at least 1x monthly.Provide instruction and assistance to participants to obtain neededcommunity support services, including medical care, mental health care,vocational services, etc. Act as a liaison between communityorganizations on behalf of Champ participants, while promoting anatmosphere of self-advocacy. Provide instruction and assistance toparticipants to complete necessary housing applications. Act as aliaison between housing organizations on behalf of Champ participantsMaintain participant housing binders. Complete required programcompliance measures, including community fee payments, alcohol/drugtesting, and room-checks, according to policies/procedures, completerequired documentation, and report noncompliance to the appropriatestaff in accordance with reporting guidelines. Address participantnoncompliance in a timely fashion, according to policies/procedures Workwith participants to address personality conflicts, while followingChamp procedures. Complete required documentation and reportnoncompliance to appropriate supervisor. On-call coverage as needed. Usediscretion and sound judgement to contact additional staff or 911 forassistance in an emergency. ADMINISTRATIVE Utilize organizationaltracking and reporting measures. Preserve Champ Homes programpolicy/procedure and program forms. Participate in trainings andmeetings, both internal and external, as directed. Work with care teamto identify barriers to meeting case coordination needs. Work with staffto develop and implement programming improvements. Participate inmulti-disciplinary and provider meetings, committees, attendsupervision, and trainings, both internal and external, as directed.Participate in Champ enrichment activities, fundraising and holidayevents that may be outside of normal working hours. Other duties, asrequired. QUALIFICATIONS: Education Experience ? Bachelor?s degreepreferred. Minimum 3 years? experience working in social services orcomparable field. Behavioral health experience and knowledge ofsubstance use disorders preferred. Bi-lingual preferred but not requiredProficient with MS Office and Outlook. Excellent verbal and writtencommunication skills with exceptional attention to details. The idealcandidate needs to be patient, flexible and able to work in a fast-pacedenvironment as a cohesive team member. Candidate must be comfortable,supportive, and able to work with a diverse population. Must have validdriver?s license.
