Summary:
The Coder I is responsible for exercising judgment in analyzing clinical documentation and accurately establishing diagnoses codes to maximize reimbursement. The Coder I appropriately applies official coding guidelines and relevant coding references to all home health, hospice, and palliative care coding scenarios. The Coder I collaborates with other teams to capture necessary quality measures. The Coder I participates in performance improvement initiatives and enhances coding knowledge and skill with continuing education.
Responsibilities:
Reviews documentation and accurately applies diagnoses codes.
Problem solves and troubleshoots documentation for: coding discrepancies, logical inconsistencies, errors, and completeness.
Collaborates with clinicians and billing staff to resolve diagnosis coding issues.
Makes corrections in electronic health record as needed.
Serves as a resource to agency staff and management in regard to the coding of patient diagnoses.
Maintains knowledge of coding and clinical practice standards by accessing various media (i.e. Instant Oasis Answers, Q&As, Decision Health, and websites) to ensure current knowledge of procedures/national standards.
Other duties as assigned.
Other Information:
Current knowledge of CPT coding, diagnosis coding and home health and hospice coding guidelines is required
Current HCS-D or equivalent certification
