　　A Brief OverviewIn collaboration with the health care team, the Clinical Nurse Specialist (CNS) Fellow provides expert population based, family-centered clinical care and clinical consultation within her/his area of expertise to improve outcomes and to promote integrated health care that is safe, equitable, timely, effective, and efficient. The CNS Fellow will be part of a centralized CNS team, and work across the organization in interdisciplinary teams.

　　What you will doManages the care of complex and/or vulnerable populations through expert direct care, care coordination and collaboration with patients and families and other members of the health care team. Demonstrates a commitment to patient safety by being knowledgeable about evidence and best practices for safety, speaking up about safety issues, and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms and communicated these with appropriate leadership. Demonstrates shared accountability for safe practice.Supports other members of the health care team, including nurses, physicians and allied health professionals through collaborative clinical practice, education, research/evidence based practice and consultation. Acts as a preceptor and mentor to support the development of nurses, students and other health care providersFacilitates change and innovation for patients/populations within the department, the health care system and the community. Analyzes and utilizes data, including patient satisfaction data, QI data, other data relevant to specialty populations and incident report/safety net data, to identify opportunities for improvement and to develop a plan of action in order to improve outcomes.Participates in scholarly activities including research and evidence-based practice. Participates in personal professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Presents research and evidence based practice findings to unit staff. Participates in regional/national programs relevant to specialty practice. 　　To carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community. As a condition of employment, professionals working onsite-at any CHOP location, for any portion of time-must be vaccinated for COVID-19.
