  A Brief OverviewIn collaboration with the health care team, the Clinical Nurse Specialist (CNS) Fellow provides expert population based, family-centered clinical care and clinical consultation within her/his area of expertise to improve outcomes and to promote integrated health care that is safe, equitable, timely, effective, and efficient. The CNS Fellow will be part of a centralized CNS team, and work across the organization in interdisciplinary teams.

  What you will doManages the care of complex and/or vulnerable populations through expert direct care, care coordination and collaboration with patients and families and other members of the health care team. Demonstrates a commitment to patient safety by being knowledgeable about evidence and best practices for safety, speaking up about safety issues, and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms and communicated these with appropriate leadership. Demonstrates shared accountability for safe practice.Supports other members of the health care team, including nurses, physicians and allied health professionals through collaborative clinical practice, education, research/evidence based practice and consultation. Acts as a preceptor and mentor to support the development of nurses, students and other health care providersFacilitates change and innovation for patients/populations within the department, the health care system and the community. Analyzes and utilizes data, including patient satisfaction data, QI data, other data relevant to specialty populations and incident report/safety net data, to identify opportunities for improvement and to develop a plan of action in order to improve outcomes.Participates in scholarly activities including research and evidence-based practice. Participates in personal professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Presents research and evidence based practice findings to unit staff. Participates in regional/national programs relevant to specialty practice. Develops departmental and hospital policies, procedures and standards based on the best available evidence.Demonstrates health care informatics competencies in order to manage patient care, outcome data and current and emerging health care technology, which includes:Basic computer competency, including the effective use of patient care devices, computers, computer software programs such as e-mail and word processing programs, and shared network file space.Information literacy, including the effective use of the intranet and internet to find electronic resources for patient care, nursing practice and evidence-based decision-making.Information management skills, including the effective use of the electronic health record and clinical informa ion systems, the ability to collect clinical data using quality tools and the ability to discuss, evaluate and analyze and present outcome data depicted in charts and tablesEducation QualificationsMaster's Degree Enrollment in Clinical Nurse Specialist Graduate Program RequiredSkills and AbilitiesEffective oral, written, and presentational skills in English.Critical thinking and problem solving skills.Ability to work across systems and to influence change.Ability to coordinate individuals and groups to develop, implement, and evaluate projects.Ability to work independently.Ability to work collaboratively.Computer CompetencyBasic use of a computer (mouse, keyboard, printer, USB ports).Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype).Web browsing, intranet search, document access.Use of shared network file space.Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors).Navigation and use of clinical communication systems.Information LiteracyDetermines the nature and extent of clinical information needed and uses the appropriate technology to access it.Accesses needed information effectively and efficiently.Evaluates information and its sources critically and appropriately incorporates it into clinical practice.Evaluates outcomes of the use of information.Information Management LiteracyNavigation of the electronic health record system.Ability to locate and review specific patient data in variousclinical information systems.Effectively uses clinical decision support tools.Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.Ability to collect clinical data using quality improvement tools.Licenses and CertificationsRegistered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required orRegistered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required andClinical Nurse Specialist (CNS) - American Association of Colleges of Nursing (AACN) - within 36 months - Required

  To carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community. As a condition of employment, professionals working onsite-at any CHOP location, for any portion of time-must be vaccinated for COVID-19.

  CHOP also requires employees who work in patient care buildings to receive an annual influenza vaccine. Employees may request exemptions for valid religious and medical reasons. Start dates may be delayed until candidates are immunized or exemption requests are reviewed.
