　　As a CNA Mental Health Counselor you need to know how to:Provide care through effective planning, intervention and evaluationMeet the needs of the patient, family, and multi-disciplinary team througheffective communication, coordination, and documentation and under directsupervision of a registered nurseCoordinate patient care and report patient changes to the appropriate teammembersEnsure that the progress of patients is communicated to staff, physicians,other departments and familyProvide patient with assigned activities and tasksDocument patients' condition in medical and facility records according topolicy and regulationsProvide therapeutic interventions on an individual and group basis which arecongruent with team structure and program expectationsParticipate in the development and implementation of treatment plans whenrequestedMaintain under the direction of the nurse, ongoing communication withinternal and external individuals and/or agencies as appropriate,utilizing effective customer service skillsEnsure compliance with applicable regulatory guidelines and requirementsDemonstrate age related care skills as defined by unit patient populations

　　High School Diploma or equivalent, requiredCurrent BLS certification endorsed by the American Heart Association or theability to obtain within 60 days of hire (with prior approval), requiredCertified as a nurse aide in the State of Colorado, required

　　Preferred RequirementCollege degree, preferredMinimum of one (1) year Mental Health Counseling inpatient experience orthree (3) years memory care / skilled nursing facility with behavioralhealth patients (as evaluated by the supervisor), preferred
