ESSENTIAL & CORE FUNCTIONS
Complete comprehensive assessments of the consumers.Using the strengths-based approach identify problems and design an ITP with appropriate and reasonable goals.Identify behavioral goals in measurable terms and selection of appropriate interventions for inclusion in the ITP.Review, analyze and interpret data to determine any changes to goals and objectives included in the ITP.Direct the implementation of behavioral interventions in the ITP.Provide consultation to BHTs on behavioral management protocols.Review of clinical outcomes for the behavioral interventions being implemented in the treatment plan with the youth, young adult, or parent or caregiver of the child to determine effectiveness of the individual services on a monthly basis.Provide individual therapyProvide family therapyAssist with crisis stabilizationComplete Progress Reviews every 90 days.Submit monthly reports summarizing program status and outcomes.Ensures outcome data is collected, reported, complied accurately and in a timely manner.Complete FBAs as needed to obtain BHT services for consumers.Implement evidence-based practices with fidelity.Review of consumer files assuring regulatory and funding standards are met and adhered to.Perform home and community visits.Serves as a liaison to adults across all areas in the child's lifeFacilitate discharge planning and implement discharge plans.Meet service productivity expectations.Complete all paperwork within specified time frames.Maintain an understanding of agency policies and procedures.Participate in and adhere to Individualized Training Plan.Attend and participate in supervision in accordance to regulatory standards.Adhere to WES's Code of Ethics and complies with State Mental Health Code.Attend trainings as required by WES.Assure continuous quality assurance/program development.Comply with WES standards for service delivery.Maintain consumer confidentiality.
