　　5W Oasis Transitional Care Unit (OTCU) is a 14-bed inpatient unit whichprovides care for patients with complex discharge barriers. Oasis has adiverse, interdisciplinary team to creatively address these barriers in aninnovative, holistic care environment. Quality treatment for patients withmultiple medical and psychosocial needs is provided by integrating nursingcare and psycho-social assessment. This team facilitates meaningful, patientactivities such as music, art and wellness programs. Each patient is treatedwith dignity and their strengths are identified in the highly personalizedcare provided. The Oasis team is high-performing, and with their innovativeapproach, they make a positive impact on patient outcomes. 　　Job Summary

　　Under close supervision, assists professional nursing and care providers inproviding care to all patients with respect for their uniqueness. Collaborateswith the RN and healthcare team. Assists with basic patient care andactivities of daily living. Maintains unit specific documentation as required.Orders, maintains and utilizes supplies for the assigned unit.

　　Essential Functions:Provides a safe environment for all patents. Assuring proper patient positionsand patient care throughout the patient's stay. (10%)Assists the RN/LPN in carrying out the established plan of care for assignedpatients. (10%)Practices sound safety and infection control practices according to hospitalpolicy and procedures (10%)Obtains vital signs and differentiates normal from abnormal findings. Reportsabnormal values or conditions to the RN/LPN. (EX. Abnormal VS,bleeding, safety concerns, family issues.)
