Family Practice & Counseling Network (FPCN/The Network) is a federally qualified health center designated to provide Public Housing Primary Care. FPCN envisions utilizing the space in the Vaux Community Building to create a comprehensive community health center to serve the residents of PHA's Sharswood community and the greater North Philadelphia area; the projected start of operations is January 2024.
Prior to the opening of the health center, a community engagement initiative will be facilitated by the Center Director, in tandem with the Director of Development, to establish rapport with residents, build a Community Advisory Council and seek partnership development. The Network is committed to providing a full range of preventive and primary health services, including medical care for all ages, prenatal and perinatal care, behavioral health/ mental health and substance use disorder services, dental services, health education, and preventive health services, such as nutrition services, transportation, social work, outreach and enrollment for health insurances, as well as complementary mind/body services. All of these services will be integrated to assure that patients receive the most comprehensive, seamless care possible.
The health center will build on the work of the FPCN school-based site that is already operating in the Vaux building. Upon opening, the Center Director will oversee and manage the operations of all services and activities at the site, upholding the mission of the organization by assuring quality care, staff retention, patient satisfaction and fiscal soundness.Responsibilities/Duties
Fosters collaboration and integration among services and to continually improve the efficiency and effectiveness of the service delivery, work environment and operations of the Center.Serves the Health Center liaison with community partners.Leads a site-level Community Advisory CommitteeIn tandem with the Directors of Primary Care and Development, facilitates the health center patient base through marketing and development in collaboration with the community and community partners.Works collaboratively with DFO, Financial Planning and Analysis Manager, and Director of Operations to ensure that operational systems within the Center are optimizing the effectiveness of billing, cash management, and other financial functions. Contributes to development of center-based budget and assumes responsibility for staying within budget limits.Supervises Shared Services Staff at the Health Center and defines job responsibilities and oversees work assignments and scheduling: Community Engagement Specialist, Social Worker, Mind Body Practitioner, Safety Advocate.
Works collaboratively with the Site Clinical Leads to:Optimize scheduling and patient flow within primary care.Develop and implement necessary workflows within primary care.Disseminate clinical practice and administrative information to support staff.Ensure required/needed trainings and competencies are completed.Ensure smooth operations in the clinical setting.Assures that required staff trainings take place such as CPR and OSHA and provides for other personal growth and education workshops as budget permits.Ensures the timely and accurate completion of required paperwork and documentation.Handles performance issues and participates in feedback with staff members and site level staff, including performance evaluations, discipline and recommendations.Works with clinical leaders to assure the dissemination of clinical practice and administrative information to support staff.Assures patient access to services.Assures that staff provides competent and culturally sensitive trauma informed anti-racist care to patients.Works with Primary Care and Shared Services teams to develop and implement new clinical and wellness programs and services based on commu ity need with collaboration with other stakeholders when applicable including data collection and metrics to analyze programs and new initiatives.Works with Compliance Manager to assure that signage and posted materials in offices provides excellent communication to patients about policies, patient services and patient education.Works with IT and Human Resources to assure the timely completion of patient and staff satisfaction audits.Performs Continuous Quality Improvement functions such as overseeing patient complaints and insures that they are addressed and documented.Supports Business Development and marketing strategies by implementing special programs and open houses, and by overseeing the preparation and dissemination of promotional and educational materials.Assures health monitors and on hold service are current and serve to educate patients about services and health care issues.Provides administrative direction and coordination of current and long-range policies, procedures and programs affecting the Center.Represents the Center in interactions with regulatory, professional, educational and community groups.Assures that building and systems are performing adequately.Oversees the inventory of supplies, acquisition of equipment/supplies and other service issues that impact on the delivery of service.Reviews invoices.Ensures compliance with RHD, FPCN, and other governmental agencies (i.e., HCFA, OSHA) policies and procedures.Assures that fire drills and active shooter drills are conducted as required by regulation and maintains all appropriate documentation.Required Experience
Bachelor's degree required. Master's in public health, Business, or Healthcare Administration or related field preferred. Required Education
Three-five years clinical and administrative experience in outpatient settings required; work with low-income communities desirable. Required Qualifications
