CHC Addiction Services is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction.We are currently looking for a CDCA/SUD Counselor for our Outpatient program. CDCA/SUD Counselors are responsible for assessing newly referred patients for various treatment needs to support recovery, by coordinating care with alternate providers, referral sources and resource engagement, to facilitate enrollment.

　　Duties include:

　　Develop treatment plansProvide patient access to individualized treatment. Monitor and facilitate progress through treatment, modify treatment as needed and inform patient of alternate treatment procedures and resources that are available.Provide counseling as indicated in the treatment plan, either in group or individual setting.Evaluate problems areas and highlight patient strengths.       Assist the patient in setting behavioral goals to prevent relapse, and improve quality of life.Monitor urine drug screens results and coordinate interventions with supervisor and other staff as appropriateAct as liaison with community groups and organizations on patient's behalf

　　Minimum Requirements: High School Diploma or Equivalent, and CDCA certification
