General Statement of Duties
This position is responsible for aligning the transformational work of service providers with the strategy and vision set by the Coordinated Care Organization (CCO). Key functions of this position include project management, contract monitoring, report writing, process improvement, and coordination and facilitation of stakeholder meetings in support of health system transformation.
Essential Position Functions
Transformation
Work closely with CCO leadership to ensure work related to transformation plan goals, projects, Process Improvement Plans (PIPs) and metrics are aligned.
Translate program improvement strategies and initiatives into a workplan; identify potential barriers and communicate with stakeholders on plan implementation.
Serve as a liaison between internal and external partners, contractors and stakeholders to facilitate collaboration and consensus building; help ensure accountability, as well as maintain technical project reports, records and documentation.
In coordination with the internal CareOregon CCO team, learn best practices and serve as a resource to clinics.
With CCO leadership, participate in creating and implementing evaluation tools for key process and outcome measures for transformation fund projects and contracts with the network.
Project Management
Implement routine to moderately complex project plans that align with the objectives of the CCO; regularly communicate project progress, timelines, and updates with the CCO team.
Mobilize project teams to optimize the network directed by policy and regulatory standards, CAP priorities, transformation fund goals, PIPs and metrics by, ensuring each clinic has ongoing and on-site technical support.
Apply improvement tools to meet transformation project goals; communicate with other departments and stakeholders as necessary.
Meeting Participation/Facilitation
Attend and participate in meetings; learn to decipher and organize meeting content.
Review data and information to help identify opportunities for programmatic improvement; develop reports and dashboards to facilitate decision making.
Work with CCO leadership and internal CareOregon CCO team to ensure all Boards, CAPs, workgroups, and department managers receive status reports needed to be adequately apprised of progress, to make decisions, and to move their deliverables forward.
At the direction of the CCO Clinical Integration Director, assist in the communication and translation of CCO and CAP strategies, priorities, best practices and updates, as well as internal CareOregon CCO team support offerings to clinic partners.
Act as a local face and voice of activities, working in conjunction with CCO team and internal and external committees.
Learn and facilitate opportunities for shared learning across CCO-funded transformation projects through integration meetings.
Participate in appropriate statewide learning collaboratives related to transformation fund projects.
Attend and serve as a resource in collaboratives.

Knowledge, Skills and Abilities Required
Growing knowledge of CCO’s and transformation within primary care, behavioral health and oral health
Understanding of project management fundamentals; growing ability to develop a formal and detailed project plan with timeline, appropriate stakeholders, and budgetary guidelines
Ability to learn how to identify and anticipate issues and act proactively to formulate plans and recommendations
Ability to lead and motivate peers and cross-functional partners and teams toward project goal attainment
Strong ability to manage multiple activities, matrixed teams, organization communications, and frequently competing priorities to implement change as necessary within time and budget constraints

Education or Experience
Required:
Minimum 3 years’ experience in healthcare or social services, including minimum 2 years in program or project coordination
Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
Preferred:
Clinical and/or quality improvement experience
Training in lean or related process improvement techniques
