  Job Summary

  The Therapy Case Worker ("Case Worker"), under generalsupervision, performs culturally competent individual, family and grouptherapy, care plan development, case management, family intervention,and referral work within Colorado OBH Standards for patients receivingpsychiatric and substance use disorder treatment at Denver Health. Subscribesto an evidence-based practice framework to manage and facilitate clinicalprocesses and progress for patients. Effectively manages a clinical caseloadand clinical documentation that spans multiple modalities of care. Goals ofclinical practice are to assist patients in their ability to effectivelymanage their psychiatric and substance use disorders, to function betterwith the community and gain stability within the community setting.

  Essential Functions:

  Patient TriageIdentifies patients' conditions, problems, and determines disposition.Interviews clients to obtain health history and/or complete intakeevaluation which may include necessary paperwork; obtains and recordspatient vital signs including temperature, pulse, respiration, and bloodpressu(40%)Patient AssessmentEvaluates emotional components, impacts of health issues and assists clientto identify strengths, dysfunctional factors and to establish detailed andcoordinated treatment plans. Makes attempts to re-engage any client who seeksa premature discharge from the program. (30%)Treatment Planning and DocumentationUtilizes knowledge of patient populations' cultural, religious,ethnic, and social systems in interactions, care planning and education.Documents counseling plan, interventions, and client response to treatmentaccording to State guidelines, ensuring that clinical supervision recordsare c

  Education:Master's Degree Required

  Work Experience:1-3 years Experience working in mental health and/or substance usedisorders, Required andExperience with client assessment and providing individual psychotherapy aswell as group therapy and documenting clinical progress throughout thetreatment episode, Required

  Licenses:BLS-Basic Life Support (BLS/CPR) - AHA - American Heart AssociationRequired andCACIII-Certified Addiction Counselor III - NAADAC - The Association forAddiction Professionals Required orACD-Licensed Addiction Counselor - NAADAC - The Association for AddictionProfessionals Required orLPC-Licensed Professional Counselor - LPC State Licensing Board Required

  Knowledge, Skills and Abilities:

  Demonstrates knowledge and applicability of the principle of growth anddevelopment over the lifespan, as well as demonstrating the ability toassess data reflecting the patient's status and interpreting appropriatecultural information of the patient(s) to whom care/service is beingdelivered/provided.Knowledge of evidence-based clinical framework.Knowledge of the disease progression of addictive disorders.Knowledge of medical terms, abbreviations, diagnosis, treatment andprognosis, and demonstrates this knowledge verbally and in writing.Ability to motivate persons for positive change.Clinical thinking skills that lead to the identification of the strengths andweaknesses of alternative solutions, conclusions or approaches to problems.Ability to provide clinical assistance, emotional support and skillsbuilding, or other necessary clinical care to patients when needed.Ability to maintain professional boundaries and confidentiality of recordsrelating to clients' treatment.Ability to develop effective therapeutic interventions that aresocial-culturall sensitive.Proficient in Microsoft Excel, Word, PowerPoint.
