Case Manager - Foundation House
JOB SUMMARY:
VOA is seeking an energetic human services professional to join our Permanent Supportive Housing team. If you’re passionate about housing for everyone, advocating for low-income and vulnerable people, and delivering data-driven social services, we want to hear from you! The Case Manager position is responsible for ensuring the housing stability of 25 formerly homeless individuals housed at a single site.
ESSENTIAL JOB FUNCTIONS:
Provide all aspects of housing case management for a caseload of 25 formerly homeless individuals.
Provide oversight of housing operations, including lease-up, resident notices and terminations, in compliance with VOA policies and relevant laws and regulations.
Develop and maintain effective community partnerships to increase resources available to the people served by VOAs housing, employment and corrections programs.
Evaluate program outcomes on an on-going basis, and work with supervisor to continuously make VOAs services more effective.
Maintain accurate records and complete data entry within specified timeframes for all clients in accordance with VOA guidelines and relevant regulatory, agency and funding source standards.
Prepare reports for the agency and funders as required.

EDUCATION & QUALIFICATION REQUIREMENTS:
Bachelors Degree in Human Services, Social Work or equivalent experience and skill sets
Minimum of two years relevant experience
NYS Drivers License
Availability to work occasional evenings and weekends as needed
Flexibility required to respond to emergency client needs
Strong organizational skills
Working knowledge of MS Office products
