Case Manager -Community Support Program (CSP) Job Code:2020-BHS-013 Location:MilwaukeeDepartment:CSP FT/PT Status:Full Time Job Responsibilities: We are afast-paced Community Based Mental Health program seeking a person withthe following qualities: Compassionate; experience working withindividuals who have a mental illness; a sense of humor; attention todetail; organized; independent worker; experience with computers. We areopen 8AM-4PM, Monday through Friday. We are pleased to offer a sign-onbonus of \$2500 to new employees hired into the CSP Case Manager role!This bonus will be paid in 3 separate checks in the following manner.\$500 after completion of the first 90 days of employment. \$500 aftercompletion of the first 6 months of employment. \$1500 after completionof the first 12 months of employment. The signing bonus is taxable, andall regular payroll taxes will be withheld. Position Summary Provide ongoingassessment and treatment planning, case management, supportivepsychotherapy, symptom management, medication monitoring, and crisisintervention including afterhours hotline coverage to clients withserious and persistent mental illness and co-occurring substance use 70%of case management is performed in the community and in clients\' homesSupport clients\' recovery through activities of daily living to includecoaching and hands on assistance in the areas of housing, moneymanagement, vocational/educational pursuits, andscheduling/transportation to appointments Work as a member of anAssertive Community Treatment (ACT) team addressing all client needswhile utilizing the ACT treatment modality Participate in groupfacilitation (as determined by supervisor) Responsible for carecoordination with clients\' natural supports Working with ACT teammatesto complete all assigned and non-assigned tasks as part of clienttreatment Assessment, referral, and assistance with client healthcareneeds Maintain frequent and...
