Completes documentation perworkflow timeline and content requirements including completion of theIndividual Plan of Care (IPoC) per CMS guidelines. Schedules familyconferences and/or communicates with caregiver following each teamconference and more often as needed to keep patient and designatedcaregiver informed of progress and provides appropriate informationrelated to goal achievement, course of rehabilitation stay, and plansfor discharge. Coordinates weekly patient care team conferences tofacilitate development, monitoring and refinement of treatment plan toachieve identified patient goals and outcomes. Reviews the patient\'sassigned CMG and helps the team identify any potential missed comorbidconditions that are actively being treated during the patient\'s stay.Communicates any findings to the HIM team. Other duties as assigned.Qualifications Current RegisteredNurseor Social Work licensure in theState of WI. Certification in Case Management or Rehabilitation Nursingpreferred; for example, Commission for Case Manager Certification (CCM);Association of Rehabilitation Nurses (ARN) certification, American CaseManagement Association (ACM) or Board Certification in CM by the ANCCe.g.: RN-BC Minimum of 2years social work or case management experiencein an inpatient setting highly preferred; acute/rehabilitation hospitalexperience preferred.
