Role Responsibilities:
As a Case Manager, you will play a pivotal role in providing support, guidance, and resources to homeless families on their journey towards stable housing and self-sufficiency. Your responsibilities will include, but are not limited to:
Conducting assessments to understand the unique needs of each family
Developing and implementing individualized case plans
Connecting families to appropriate resources, services, and programs
Monitoring progress and providing ongoing support
Collaborating with a multidisciplinary team to ensure comprehensive care
Requirements
Requirements:
Must have experience with a Coordinated Entry System (CES)
Education: Bachelor's degree PREFERRED in social work, psychology, counseling, or a related field.
Experience: Relevant experience in case management, social work, or a related field. Experience working with individuals experiencing homelessness is often preferred.
Communication Skills: Strong communication and interpersonal skills to effectively engage with clients, colleagues, and external partners.
Empathy: Compassion and empathy towards individuals facing homelessness, recognizing their unique challenges and strengths.
Problem-Solving: Ability to assess complex situations, identify barriers, and develop effective solutions.
Cultural Competence: Sensitivity to diverse backgrounds and cultures, with the ability to work with individuals from various walks of life.
Teamwork: Ability to work collaboratively with a multidisciplinary team to address clients' needs comprehensively.
