Description
Pay $19.00 - $21.00 an hour
The Case Manager is responsible for conducting and scheduling assessments, providing referrals, advocacy and counseling, educating individuals and team members, ensuring compliance with state, federal and Agency regulations and operating standards, completing all records and reports, participating in Agency activities and providing transportation as required.
CORE RESPONSIBILITIES
Conduct and schedule assessments, evaluate needs, and develop treatment planning processes for adults, children and families.
Develop and maintain an effective communication network with the individuals, program staff, families community agencies and other Agency departments.
Provide referrals for individuals and/or their families to appropriate services.
Provide advocacy, counseling and support to individuals and/or their families as needed.
Educate individuals and/or team members about results of diagnostic evaluations or treatment planning and/or other information pertinent to the treatment team.
Complete all required documentation according to program regulations.
QUALIFICATIONS
Associates Degree in Human Services, Sociology, Psychology or related field.
1-2 years of related experience working with persons with a disability.
Knowledge of basic computer programs (Including all Windows programs, Adobe Acrobat) are required.
Must have a valid NYS Driver’s License.
