　　Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management.

　　Required Qualifications:

　　Master's Degree in Social Work or Related Field.

　　3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.
