Description

　　Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

　　Required qualifications:

　　Master's Degree in Social Work.

　　Coursework/Training and All LCSWs must follow the guideline by Los Angeles County Department of

　　Mental Health for LPS designation and keep their designation in current status.

　　Upon hire: California Clinical Social Worker License

　　Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluations

　　Within 1 year of hire: CPI certification (nonviolent crisis intervention training)

　　2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.

　　Computer experience.

　　Preferred qualifications:
