OverviewThe Starting Point Care Facilitator plays a crucial role in coordinating and managing the care of patients. They ensure effective communication, continuity of care, and optimal patient outcomes. They work closely with healthcare providers, patients, and their families to facilitate a patient-centered approach to healthcare delivery by fielding triage calls and coordinating care across services.
ResponsibilitiesPatient Engagement:
Respond to incoming patient and family questions and escalate emergent triage calls to the clinical team
Provide timely, accurate, and helpful information to patients and support the overall care goals of our programs by proactively contacting patients for longitudinal care managementTreatment Coordination:
Coordinate and schedule appointments and referrals to ensure timely and appropriate follow-up careCommunication and Collaboration:
Facilitate effective communication and collaboration among healthcare providers, patients, and their familiesServe as a liaison between services and external providers to ensure thorough care coordinationMonitoring and Evaluation:
Monitor patients' progress and response to treatment, identifying any changes or complicationsCommunicate escalations or new health risks to clinical leadershipDocumentation and Record-Keeping:
Maintain accurate and up-to-date communication logs. Adhere to documentation standards and regulatory requirementsQuality Improvement:
Participate in quality improvement initiatives and performance improvement projectsIdentify areas for improvement in care coordination, patient outcomes, and patient satisfactionImplement strategies to enhance the quality of careCompliance and Regulations:
Adheres to HIPAA regulations and maintain patient confidentialityEnsures compliance with clinical guidelines and standards of care
QualificationsSome College or associate degree in clinical sciences, Nursing, or a related fieldMinimum two (2) years of experience collecting referral information in the healthcare marketMedical Assistant certification preferred, but not requiredExperience with electronic medical record software and proficient use of the Microsoft Office suiteAbility to multitask, prioritize, and manage multiple projects simultaneouslyExcellent written and verbal communication skills and ability to work independently or in a team setting
About our Line of BusinessStarting Point Nursing Services is staffed 100% by a team of remote Registered Nurses trained to support the Post-Acute Continuum with clinical advice and interventions. Our 24/7 On-Call and Triage team responds to Physician Practices, Home Health Agencies, Intellectual Disability Providers, and Assisted Living Providers. Our services are tailored to our partners, including remote physical/chart assessments, predictive care modeling, clinical programming interventions for high-risk populations, emergent care coordination, and case management.
