　　Vaccination for COVID-19 and Influenza (flu) is mandatory for all MSK staff. Staff are considered fully vaccinated upon completion of a primary vaccination series for COVID-19 (i.e., one dose of a single dose vaccine or a final dose of a multi-dose vaccine series). Exceptions from the COVID-19 and Influenza (flu) vaccine requirements are permitted for those who request and receive an approved medical, religious, or fully remote exemption. Requests for exemption can take up to two weeks to review. Vaccination or an approved exemption is required to start work.

　　We are looking for Care Coordinators in ourNassau, Commack & Hauppaugelocations in Long Island.

　　You Will:

　　Work as a member of the practice staff team of Care Coordinators to ensure that each physician practice session is well-prepared, and organized and that each patient visit occurs with satisfaction

　　Monitor clinic schedules and revise as necessary to facilitate smooth patient flow

　　Serve as a liaison between the patient, family, and clinical team during the office visit

　　Be responsible for expediting time-sensitive scheduling (within 48 hours), (e.g., same-day tests or consultations) by calling the appropriate areas to ensure timeliness. Coordinate inpatient admissions, if needed

　　Reconcile work after clinic and complete a detailed End of Clinic summary daily

　　You Are:

　　An effective communicator, capable of determining how best to reach different audiences and implementing communications based on that understanding

　　Adept at building partnerships and working collaboratively to meet shared objectives and goals

　　Flexible in your approach and demeanor in order to align with the shifting demands of evolving circumstances

　　Passionate about working in a healthcare setting and committed to delivering a best-in-class patient experience

　　You Have:

　　Experience in customer/patient service is strongly preferred

　　A proven dedication to healthcare through work, internship, or volunteer experience

　　Ability to handle acutely ill patients in stressful situations with a positive demeanor

　　Strong digital literacy

　　Hours vary between 6:15 am and 10:00 pm (Monday-Friday, occasional weekends) . Shifts are 12.5 hours, 3-4 days/week.
