　　Care Coordinator

　　(Full-time, 40 hours/week)

　　Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.

　　JOB SUMMARY:Under the direction of a designated supervisor/manager, the Care Coordinator provides comprehensive support services and accelerated access to care, in order to improve health outcomes and support patients with complex health conditions and needs; assists with the development and ongoing continuity of collaborative relationships with community sites and programs for designated health center services and programs; provides community health outreach and other support services to targeted populations to improve awareness about designated program(s) and resources (e.g., medical, oral health, mental health, prenatal, family, etc.). May participate in health education activities and events.

　　Qualifications

　　Bachelor’s degree in human services or related field or equivalent related course work and work experience desired. Minimum of two (2) years care coordination/case management experience. Valid driver’s license and availability to properly insured vehicle are essential throughout employment.

　　Pay Rate: $18.25/hour
