  Description

  Venice Family Clinic is a leader in providing comprehensive, high-quality primary health care to people in need with compassion, dignity and respect. In November2021, Venice Family Clinic merged with South Bay Family Health Care, uniting more than a century of experience helping patients regardless of their income, insurance or immigration status. The organization now has more than 500 staff who serve 45,000 people from the Santa Monica Mountains through the South Bay.   With direction and guidance from the Director of Care Management, the Care Coordinator provides high quality care coordination to Venice Family Clinic patients.

  The Care Coordinator will:

  Reach out to patients who have rising risk, high risk and/or social needs and provide care coordination services.

  Conduct a comprehensive high risk assessment with each patient including demographic information and biopsychosocial history.

  Act as patient advocate, and provide intervention and support to patient when appropriate.

  Connect patients to resources for basic needs, and provide appropriate information regarding referrals (ensuring linkage).

  Assist patients with scheduling appointments and coordinating transportation to appointments.

  Provide disease specific and preventive care patient education.

  Provide administrative support and complete other duties as assigned.

  Salary Range: $29.64-$39.13 HourlyQualifications

  Required:

  Minimum of 2 years of experience in a health care or social services setting working directly with patients/clients.

  Knowledge of community resources, health education methods, advocacy, harm reduction and general clinical knowledge.

  Bilingual in English and Spanish (oral and written language skills).

  Ability to complete required program documentation, including the use of electronic health records and other software.

  Ability to create and maintain boundaries with clients.

  Ability to follow through on assignments, prioritize, and work effectively with people of diverse cultural and educational backgrounds.

  Preferred: College degree OR equivalent care coordination experience OR licensed as a vocational nurse (LVN).
